Q I think it will be helpful for you all to review an accident scenario, with the intent of helping you understand how many aviation accidents happen. While this is not a "flight safety" course, because our topic is managing aviation safety, it is still important for you to get a sense of the error chain that is typically experienced in the lead up to an accident. Very rarely is an accident "cause" a singular event. There is typically a trail of problems that culminate in that event. The key to understanding this is to keep questioning "why" a particular operator error was committed. When you get to these root causes, then, as managers, you can start working on eliminating those causes. So, with all that said, after viewing the American Airlines Flight 587 video, please discuss not only what you believe the root causes of the accident to be, but what could be done by all the players involved to try to eliminate these causes. Pay particular attention to the "organizational causes" of this accident. Please note - there's a lot of "root causes" to go around, and no one person needs to try to cover them all. Just pick one area to discuss initially, and then build on each others' posts.

I think that the root cause of the accident had been the way in which trainee pilots had been trained to use the rudder in an aggressive manner in case of emergencies. The emergencies could be turbulent wind forces or any other air forces in the sky. However, the trainee pilots had not been taught that using the rudder violently or aggressively would be the cause of making the tail of an airplane more sensitive in situations of emergencies. The trainee pilots should have been taught that the rudder is a very sensitive part of an airplane to be used and therefore, pilots must use the rudder sensibly even in emergency situations. The manufacturer of the aircraft of American Airlines did not have the idea that the rudder had been sensitive and that the rudder could be used aggressively.
